Post-Acute / Skilled Nursing and Rehabilitation Centers
Post-Acute and Skilled Nursing Facilities (SNFs) both provide the same services with increased emphasis on rehabilitative therapies for convalescent patients. Covenant Care facilities offer both skilled short-term and long-term care under the direction of our Medical Directors and treating physicians.
"In terms of rehabilitation in skilled nursing facilities, hospitals make the arrangements for follow-up patient care after an acute hospital stay, like after a surgery. When released from the hospital, a patient transfers to the skilled nursing home to receive hands on care from nurses. If a patient needs rehabilitation like physical or speech therapy, a patient receives the services until they're able to go home."

Residential Care / Assisted Living Centers
Residential Care or Assisted Living Facilities help provide a special combination of residential housing, personalized supportive services and healthcare. These residential settings maximize independence but do not provide skilled nursing care. These facilities services are designed to meet the individual needs of those requiring help with activities of daily living, but do not need the skilled medical care provided in a nursing home.
Covenant Care residential care facilities are located on the same land as a skilled nursing center to provide a broader range of services to its residents.

Affirma Rehabilitation
Affirma Rehabilitation is the rehabilitation therapy division of Covenant Care that provides diagnostic treatment for conditions like Orthopedic Injuries, Strokes and Aneurysms, Neurological Conditions, Cardiopulmonary Conditions, Knee and Hip Replacements, Arthritis, Amputations and Prosthetics, Speech and Swallowing Disorders, Cognitive and Memory Problems, General Deconditioning, Wounds, and Pain.
All Covenant Care facilities, as well as a number of non-affiliated facilities and home health agencies, receive quality, innovative, results oriented physical, occupational and speech therapy services from AFFIRMA Rehabilitation.
Elevate Home Health
In 2013, Covenant Care became a Home Health Service Provider in strategic markets, enhancing continuity of care for the patients.
